Beaver Dam Unified School District Student Health Services follows Wisconsin State Immunization Laws. Parents are required to provide the schools with an accurate vaccination record indicating their child has the vaccinations required for their grade. Parents may also sign a waiver of the requirements based on medical (also requires health care professionals signature), personal, or religious reasons.
The Wisconsin Immunization Registry (WIR) is not linked to the BDUSD health record keeping system. If parents do not have an immunization record for their child, they visit WIR website to check if vaccines provided by Wisconsin health care providers, including the public health departments, are listed and provide the school with those dates.
THE DATES MUST MEET STATE IMMUNIZATION LAW REQUIREMENTS OR PARENTS SHOULD 1) GET CHILD IMMUNIZED, OR 2) SIGN A WAIVER.
State law requires all public and private school students to present written evidence of immunization against certain diseases within 30 days of admission.
