Beaver Dam Unified School District - 5 Year-Old Kindergarten Enrollment

Enrollment for 5-year old Kindergarten can be completed at the Educational Service Center located at 400 E. Burnett Street, Beaver Dam.

2026-2027 School Year Enrollment

Enrollment is only for students NOT currently enrolled in BDUSD's 4K program. Families of children enrolled in our 4K program will receive separate information regarding kindergarten enrollment.

5 year-old Kindergarten enrollment for the 2026-2027 school year will be held at the Education Service Center (400 E. Burnett Street) on the following dates/times:

No appointments are necessary.
• March 2	2:00 pm - 7:00 pm
• March 3	7:00 am - 2:00 pm

Children 5 years-old on or before September 1, 2026, are eligible for 5 year-old kindergarten.

Please bring the following information for enrollment:

Completed enrollment forms
Birth Certificate (original/certified from state/county)
Current Immunization Record
Proof of Residency (see list below for required documents)

Proof of Residency Requirements

TWO (2) documents are required to prove residency (one (1) from each category)

CATEGORY A

CATEGORY B

Current accepted purchase agreement, closing statement after purchase of the home, or mortgage statement.
Current lease agreement. Month-to-month lease agreements must be dated and valid on the date of the enrollment request.

Current billing statement. Acceptable billing statements include bills for gas/electricity, water, cable/satellite, landline/cell phone, or credit card.
Current wage statement or bank statement.
Auto, Health, or Homeowner's/Renter's Insurance statement.
Government correspondence for Supplemental Security Income (SSI), Wisconsin Works Cash Benefits (W2), Temporary Assistance for Needy Families (TANF), Food Share letter, or Housing Assistance letter.

5K School Assignments:

School assignments for district resident students will be based on the school assigned to your address, special programming needs, and enrollment numbers. The Beaver Dam Unified School District will determine all final placements.
