The Beaver Dam Unified School District offers an exceptional educational environment for students, involving them in academic projects, co-curricular activities and community service opportunities through the school year. Student growth and achievement of all children is the vision of our school district.
Beaver Dam offers half-day 4-year-old kindergarten and all-day 5-year-old kindergarten classes. Starting children off right - being prepared to meet their individual needs, understanding their individual personal development, and preparing to work with and for them as they grow and learn - is the primary goal of our early childhood and kindergarten programs.

Enroll your new student by following these steps:
STEP 1: Determine the student's eligibility
Enrollment in the Beaver Unified School District is open to all school-age residents within the established boundaries of the Beaver Dam Unified School District and to those students from other districts who have been accepted through Open Enrollment options.
Children age 4 on or before September 1 are eligible for 4K and children age 5 on or before September 1 are eligible for kindergarten enrollment.
STEP 2: Register
- All elementary-age students (4K - 5th grade) register at the Educational Service Center (400 E. Burnett St.), 7:00 am - 4:30 pm, Monday - Friday
- Middle School students (grades 6 - 8) register at Beaver Dam Middle School (108 Fourth St.)
- High School students (grades 9 - 12) register at Beaver Dam High School (500 Gould St.)
Parents registering their children in the Beaver Dam Unified School District must provide the child's proof of residence and proof of immunizations. 4-year old and 5-year old Kindergarten students also must provide their birth certificates, and some grade levels require additional forms as shown below:
- Proof of residency (all grades)
- Proof of immunizations (all grades)
- Birth certificate (4K and Kindergarten)
- Additional forms that may apply (examples: asthma/allergies, medication)
Proof of residency is required when registering your child in the Beaver Dam Unified School District. Two documents are required to prove residency in the district. One document from each of the two categories below. All items must have the name of the parent/guardian at the address listed on the enrollment form. Items in Category B must be dated within 30 days of the date of enrollment.

- Current accepted purchase agreement, closing statement after purchase of the home, or mortgage statement.
- Current lease agreement. Month-to-month lease agreements must be dated and valid on the date of the enrollment request.

- Current billing statement. Acceptable billing statements include bills for gas/electricity, water, cable/satellite, landline/cell phone, or credit card.
- Current wage statement or bank statement.
- Auto or Health Insurance statement
- Government correspondence for Supplemental Security Income (SSI), Wisconsin Works Cash Benefits (W2), Temporary Assistance for Needy Families (TANF), Food Share letter, or Housing Assistance letter.

Immunizations: Wisconsin state law (252.04) requires that all students be immunized against diphtheria, tetanus, pertussis (whooping cough), polio, measles and rubella. Written evidence of completed basic and booster immunizations, including the day, month, and year is required. This requirement will be waived for an individual student only for health, religious, or personal conviction reasons. If students are not fully immunized upon admittance to school, they must have received at least one dose of each vaccine required for their grade within 30 days of admission. They must remain on schedule, and complete all required immunizations within one year.
